10 pages] This handout was developed by Simone DeVore, an assistant professor in the early childhood education (dual certification program) at UW Whitewater. She has been working with the Department of Communicative Sciences and Disorders at UW-Whitewater, Technical Colleges in Waukesha and Beloit and area preschools (Wee Know Schools in Hartland, Kids House and Janesville Community Daycare in Janesville), and the Wisconsin Department of Public Instruction to demonstrate the use of Routines Based Interviewing, Integrated Therapies and Embedded Intervention. These practices are part of the National Individualizing Preschool Inclusion Project at Vanderbilt University. This handout provides some of the step by step processes to develop and maintain a successful relationship as you provide integrated services through consultation. Adaptations to Proposed Environments [97K ·
1 page] This tool can be used in the IEP process for placement decisions.
In particular, if the IEP team determines that it is not probable for the
child to master goals and objectives already identified in the IEP mainstream
setting without modifications, the team should discuss whether the environment(s)
should be modified. This form will assist with a list of factors which should
be considered in making this decision.

Early
Childhood Community-Based Services Form [7K ·
1 page] This form was adapted from a document developed by the Madison
Metropolitan School District for use in considering preschool options (Least
Restrictive Environments) for young children with disabilities. This form
may be used in a variety of ways and completed by various individuals:
- This form will be very helpful in gathering the information necessary to complete the Early Childhood Environment Code for the third Friday in September report.
- Families or Birth To 3 could complete the form before the IEP meeting and utilize it when discussing the child’s present level of educational performance in his or her natural environments.
- This form could be utilized with the family during or following the Transition Planning meeting. A transition coordinator or other personnel from the public school could gather information that will guide or define further assessment needed.
- This form could be completed at the IEP meeting when considering the child’s current environment as a location where special education services could be delivered. In this case the form may be attached to the IEP or it may be used as a separate worksheet, which is not attached to the IEP.
- This form would provide a tool for districts to be more sensitive to the family needs, interests and skills. This process would mirror that of an IFSP to the family of a child transitioning from Birth To 3.
- Children and families may have many regularly scheduled events (such as therapy, medical appointments, community activities, library story hour or playgroup and others). The family could complete this form to communicate the detailed schedules their children may experience before school services begin.
